How to use this worksheet

Identify the exact moment

Note the rehearsal date, actor and character, act or French scene, and page or cue. Record the cue immediately before the line so the moment can be located without guessing.

Write what happened, not a judgment

Capture the expected text and what was heard or missed as accurately as possible. Use a neutral note type—pickup, substitution, skipped line, prompt or other—and avoid comments about the performer.

Follow the production's note process

Check whether the director wants notes delivered privately, at notes or in a separate session. Distinguish a spontaneous miss from an approved script change, and update the record if text is revised.
