How to use this worksheet
Set up before doors open
Add the production title, audition date and the person who will handle forms. Decide which questions are required, and tell performers how their information will be used.
Keep check-in moving
Give one form to each performer. Ask them to print clearly, mark every conflict date and leave any role they are open to blank rather than guessing.
Review after auditions
Transfer only the useful details into your casting records, check names and contact details, and store completed forms somewhere access is limited.
