Stage manager rehearsal report template

A clear report gives the director, designers and production team a shared record of what happened and what needs attention next. Capture decisions and owners while they are fresh, then send a concise, dated summary after rehearsal.

Best for: Documenting rehearsal progress, production notes and assigned follow-ups. Download the PDF to print, or upload the companion CSV to Google Sheets in your own account.

The PDF contains a filled illustrative example. To use the companion CSV in Google Sheets, upload it to a spreadsheet in your own account. These are downloads, not hosted Google templates.

How to use this worksheet

Open with the facts

Record the date, call and release times, room, rehearsal focus and attendance. List absences or late arrivals factually, following your organization’s privacy practices.

Separate decisions from requests

Summarize scenes or sequences worked, then record each change, question or outstanding need as its own note. Avoid paraphrasing a discussion as an approved decision unless it was confirmed.

Assign a next step

Give each follow-up a named owner and due date when possible. Distribute the report to the agreed production list, and carry unresolved items into the next report.
