How to use this worksheet
Explain the purpose and get agreement
Tell the performer how measurements will be used, who can see them and how long the record will be kept. Make participation and any optional fields clear before measuring.
Use one unit and consistent method
Choose inches or centimeters for the entire form. Measure over the intended fitting layer with a flexible tape, keep it level, and record the unit beside the value.
Check and protect the record
Read entries back to the performer, date the fitting and identify the measurer. Store the sheet securely; never include measurements on a cast contact list or public document.
